To register, please complete the Intended Parent registration form available online after you have created an account on our website.

We will provide you with a power of attorney (POA) that authorizes our attorney to sign all necessary medical documents for the clinic, as well as the surrogate mother agreement. Please ensure you send us high-quality scanned copies of both your and your husband’s passports, along with a certified copy of your marriage certificate issued by the State. Please note that religious marriage certificates are not valid without a State-issued marriage certificate. After we receive the copies of your passports via email and you have completed the online registration form, we will proceed to send you the POA.

  • The Ukrainian government mandates that these three documents; passports, marriage certificates, and power of attorney—must be notarized and affixed with an apostille certification. An apostille is an official certificate that verifies the authenticity of documents, both original and copies, for use in another country. You can get an apostille from the Office of Consular Affairs at the Department of State. For citizens of Europe or Scandinavia, you should check with your local court for the apostille procedure.
  • Arrange a meeting with a notary, ideally one who can also facilitate the expedited processing of Apostille certificates. It is important to remember that both your signatures are necessary on all pertinent documents.
  • The signing of the Power of Attorney must occur in the presence of the notary. Please be aware that the Ukrainian government does not accept electronically signed documents. We will, however, accept digitally signed copies until you have dispatched the original documents via an international courier service like DHL.

Based on the surrogate plan you have chosen, your husband will be required to undergo several blood tests for sexually transmitted infections (STIs), along with a few standard genetic tests and a current comprehensive semen analysis and culture. If you are utilizing cryopreserved embryos, we will need the test results from when your embryos were created. For those who have selected a self IVF cycle surrogate plan, a list of necessary tests will be provided to you.

We will send you with all essential details about your surrogate mother. There is no selection process involved, as the primary consideration is her health and capacity to conceive and deliver a healthy baby. If your plan involves an egg donor, we will provide you with a curated list of suitable young donors based on your blood type and specific characteristics.

We advise that your husband begin taking Coenzyme Q10 (CoQ10) at least two and a half months prior to a speriogram (analysis) and the collection for fertilizing your eggs or those of an egg donor. CoQ10 is a naturally occurring compound in the body that contributes to sperm health and may enhance sperm quality in men experiencing fertility challenges. Research indicates that CoQ10 can improve sperm motility, morphology, and may also decrease DNA damage, thus increasing the likelihood of successful fertilization and pregnancy.

If you intend to begin your Serenity Surrogacy program earlier than two months, that is acceptable as well. The advised daily dosage is 600mg until the collection of your husband’s sperm. If you have sufficient time, the suggested dosage ranges from 200mg to 600mg per day of Q10. It takes roughly 74 days for sperm production to occur in the body.

  1. In Slovakia, we collaborate with a fertility clinic located in Bratislava. We can arrange an appointment for you to have your sperm collected there, which will then be sent to us in Kyiv. There is a fee for this service, and we will share the cost details during a consultation.
  2. In Uzhhorod, Ukraine, is situated in the western part of the country, bordering Slovakia and in close proximity to Hungary, just 374 km from Budapest. At our partner clinic, we will collect both your husband’s blood and sperm. With the blood sample, we can conduct the necessary genetic tests and also screen for STIs as mandated by the Ukrainian Ministry of Health. This option is more economical, costing approximately €500. Furthermore, crossing the border into Ukraine provides documentation to aid in the DNA matching process prior to the issuance of your newborn’s birth certificate.
  3. In Warsaw, Poland, a representative from The Embryon Family can meet you at our partner fertility clinic to collect your sperm. In this scenario, we have the capability to hand deliver both fresh and cryopreserved sperm if your program necessitates it. We will provide a quote for this option upon request.
